zswap/swap 修复：丢弃零页陈旧 zswap 项，修复跨 swap 文件 extent 的大页换入，并调整同步换入停顿统计。

mm/zswap: drop the stale zswap entry of a zero folio
当 swap_writeout() 发现 folio 已全零时，除设置 slot 零标志外还要删除该 swap slot 的旧 zswap entry；否则 swapin 后改写为零并回写会留下陈旧压缩数据，shrinker 可能把旧内容写回。

mm, swap: fix large swap-in across swap file extents
在 thp_swapin_suitable_orders() 中要求 swap area 为 SWP_BLKDEV 才允许 large swap-in，避免 swap file 相邻 slot 跨 extent 时单 bio 从首槽扇区读入错误数据、泄露其他文件内容。
