mm: recognize PMD swap entries in the softleaf layer

softleaf层接受PMD swap条目并新增pmd_is_swap_entry()；pmd_softleaf_to_folio()对无PFN条目告警返回NULL，softleaf_from_pmd()还需剥离exclusive覆盖位。
