selftests/mm: add PMD swap entry tests

selftests/mm新增PMD swap条目测试，fixture先MADV_PAGEOUT并回收，用mincore判断cache是否仍驻留；覆盖swapin、fork、write、munmap、mprotect、mremap、mincore、swapoff等并在条件不满足时skip。
